For your work card, either last name is fine. For your green card, hopefully you listed your married name as primary. That means your green card will have your married name on it. Otherwise you can request at interview and the officer may put in that request for you. Either way, you can get other government issued documents in your married name by showing your original marriage certificate.

If she entered legally, no problem. If she entered without a visa, it's complicated but not impossible. If she entered illegally, the first step once you're 21, is to file the I-130 packet.
