U.S. will not issue a visa without a passport. Your fiance can request a passport from the Embassy of Afghanistan in Sweden. He can also contact the United Nations' High Commissioner for Refugees, which might help him in many ways, including providing an identity document if the Afghan Embassy in Sweden would refuse to issue a passport.

If you are a USC then you can file for Adjustment of Status (I-485) along with an Petition for Alien Relative (I765). You will also need forms G325, I765, I864, pictures, proof of income, birth-certificate, proof of citizenship, and proof of marriage.
