Applying for a Reentry Permit is an option but you must do this while you are physically present in the U.S. and remain long enough to be fingerprinted. As a Permanent Resident you are obligated to pay taxes on your worldwide income. I suggest you contact an accountant or tax attorney experienced in international tax issues to handle your returns for you as they can advise you on any tax treaties as well so you aren't paying more taxes in both countries than necessary.

You should seek advice from a general practice attorney but my recollection is that you need to file a Petition in the State court after advertising for a considerable period of time (advertisement must comply with the requirements of the statute). Then you submit the name change order to the Judge and with this new Decree you can correct all of your other documents.

If you left the US at the age of 16 and did not return until you were 18, you will not be eligible for DACA because you will not be able to establish continuous physical presence in the US from the age of 15 to present.
