What are the dates of your I94? If you overstayed in this country, you should not leave US, because you might trigger 3/10 year inadmissibility bar. If you did not overstay your I94, and plan to travel to your country, get married and go back, it's ok. However, if you plan to travel to your country and come back to the US with the intent to marry, you will be committing immigration fraud.

The fact that your wife is receiving or will receive Medicaid should not affect your petition. However, the real question is whether your wife can financially qualify to sponsor you without a co-sponsor. I strongly suggest that you contact an experienced immigration attorney for a face-to-face consultation and give him/her all of the facts surrounding your situation. He/she would then be in a better position to analyze your case and advise you of your options.

Your spouse can apply with you for the green card and should be able to work on the derivative green card he/she gets. We presume you listed your spouse on the Application when you made it.
