The primary reason anyone is ever asked to leave the home is physical or emotional domestic violence. It may be that your husband thinks it would be better for all concerned if he left the house. You should probably talk to a family law attorney in your county to protect your rights.

You need to find a lawyer who does QDROs, that is drafts a Qualified Domestic Relations Order. You can't get it done without one. You need to join the plan to the case for the court to make orders against the plan. Then you will need the QDRO to order the plan administrator to do what needs to be done.

You will need to call your ex ask him one more time to sign the papers as ordered by the court in your decree; if he refuses, you need to go back to court on an Order Shortening Time for an Order whereby the court can sign for him and request attorney fees for the appearance and motion if you use counsel. My position on the who should draft the QDRO is he should be responsible, he's getting the benefit.
