Contact the police and file out a criminal report. They will investigate. If evidence is sufficient, he could be charged with stalking. You can also go to court and seek out a civil anti-harassment order.

you should notify the therapist, and she may want to call the police.  The doctor/patient confidentiality is a lot like attorney/client in that anytime a patient or client is threatening harm to self or others, you can report that.  I would definitely tell the therapist, and indicate that you believe that he will go through with this threat, and ask what to do if he does attack.

I would work closely with the prosecutors office. The prosecutors office has a right to subpoena you. If you are subpoenaed as a witness, you must appear in court. The judge wI'll hold you in contempt  if you do not appear. However, if you are in danger I would put that on the record with the prosecutor to protect yourself. You will protect yourself with both the prosecutor and the judge if there is a written record of the danger you are in.
