Why would they send a request for notice if the case is a clear no asset case?

You are assuming that your creditors know that you "clearly have a no asset case." Many times trustees will find more assets in a pro se case than in a case where the finest attorney is hired to represent a high income debtor. And if you left a creditor off of the matrix, you can blame the creditor to ask for notice from the court. The creditor has the legal right to receive notice that you appear to be denying it. And it is not like it costs a creditor anything to file this document.

It can stay up to eight years. It should show BK as a comment to indicate the debt was part of a bankruptcy filing.

Does a US citizen file a medical bankruptcy since he was not been able to pay his medical debt when he got sick while traveling to Canada?

There is no such thing as a medical bankruptcy. There is just a bankruptcy. A bankruptcy filed in the US will stop Canadian Creditors from pursuing you here. Whether they can or will try to pursue you in Canada is a different and much more complex question. I have no idea what sponsorship of a child is so it is impossible to answer that question.
