I assume you are referring to insuance. You would have to review this with your agent to determine exactly what your policy says. I would argue that this is not a "chargeable accident", meaning that your rates should not go up, for the reasons set forth in your question.

If older brother signed authorization for move her hr should know where they sent her. He should have read the authorization form and asked for a copy when he signed. Now you're stuck with all kinds of Federal and state regulations prohibiting disclosure of patient information without proper authorization (e.g. HIPAA).
