If I have a major medical issue without insurance and am willing to make small monthly payments, can they seize my house?

By "they" I am assuming you mean the hospital or doctor or other provider of medical services. They cannot merely seize your house. They can file a lawsuit against you and, if they win, they can place a lien on your house and might be able to force you to sell your house to satisfy the lien. However, there are a number of legal things you can do to prevent this from happening, and in order to properly defend yourself you need to consult an attorney as soon as possible.

I came across your question and saw that no one has posted a response yet.  If you are a homeowner and carry homeowner's liability insurance, you should turn this matter over to your carrier to take care of for you.  Your homeowner's insurance carrier will provide you with an attorney if you have been sued (duty to defend) and will pay any damages that you are liable for up to your policy limit (duty to defend).  Who told you to mediate within 30 days?  Has a case actually been filed against you in a court?  Are you representing yourself pro se in the case?
