It does not really matter, as long as you can prove who you are. You generally want to be consistent, however, so as not to confuse people regarding your identity.

Can anyone please advice me if it is legal to have this Last Will form from Office Max being filled out or if I have to get an attorney to do it?

Answered by attorney David L. Leon
Estate Planning lawyer at David L. Leon, P.C.
I make a decent living cleaning messes created by DIY wills. My advice- get a lawyer to write your will. Most wills are inexpensive. Many charities will do them for free or for a reduced fee if you are in hospice care/terminal/etc.

I want to sell my parents house but my mother refuses. What options do I have?

You can petition a court to break the trust but you'd probably still need your mother's permission to sell the property if she's the named beneficiary. Or you can let the house be foreclosed upon by the mortgage companies who will sell it and use the proceeds to pay off as much of the mortgages as possible.
