There are no forms. You need to create them, or get them from an attorney. Still trying to deal with it yourself, even though you have already messed it up, is that correct. You are going to work a jail sentence for yourself.

Probably not, the conversation as you relate it may be helpful, the fact that the Officer had recording equipment available and chose not to use it may be raised during either your trial or other confrontational hearing.

A good, experienced DUI attorney knows what he's doing and will have a good reason for whatever he asks you to do. It could be for a variety of reasons. It could be so he has an idea how your assessment will turn out as well as helping your prepare for the one you will eventually take with the court. It could also be so that if you have a really good assessment that it could be used as leverage in the plea bargaining stage to get a better deal.
