My friend was arrested for DUI but her Miranda rights weren't read. Can that affect the outcome of the case?

Most likely it will not affect the outcome of the case. Miranda is only required if the questionings occur when the person is in custody. Courts have held that routine questions at a traffic stop or DUI stop do not require a Miranda warning.

Is there any way to get the DMV to consider the time I already was restricted and not add the extra time and fee?

It is all fair, because driving is a privilege, NOT a right. You committed the DUI. You wrote you were arrested for a DUI. You never wrote you were convicted. Even better, you never wrote that you committed the DUI. In reality, YOU COMMITTED THE DUI, and put everyone else on the road that day in danger. Was that fair to the two year old, the 22 year old or the 72 year old, none of whom had anything to do with your drinking. Those are the folks the DMV are concerned with not you. You did not think about those people, so the DMV is not overly concerned with you.

What makes the dui extreme? If it's blood alcohol, a first dui in California carries a minimum 2 days county jail a varying fine depending on which county you're in, and a dui school between 3 and 9 months. If the dui is extreme because of a car accident or injury you could be facing a felony which could be probation and jail time, or 16 months 2 years or 3 years in a state prison.
What makes the dui extreme?
