Well, there are not any forms. You would have to hire a lawyer to do it for you, or go down to the County Law library and figure it out.  Everything you would need to do it on your own is down at the law library. Good luck!

Without an attorney I would say your chances are slim to none. Consult with an attorney. DUI convictions are far reaching and even without an attorney can result in costs to you in excess of $10,000.00 due to fines, increased insurance, alcohol treatment, loss of license, and impact on job.
