It is your responsibility to provide the IRS tax returns. You may request a transcript for free. A transcript is not the return itself but it provides the information from the filed return and it is an approved substitute.

It's too late to reaffirm your mortgage now. It's generally not a good idea to reaffirm a mortgage in a California Chapter 7 anyway (although there are rare exceptions). Most knowledgeable bankruptcy attorneys would consider it malpractice to recommend reaffirming a mortgage. While it is true it would normally be a violation of your bankruptcy discharge for your lender to contact you about restructuring, refinancing or modifying your home loan after bankruptcy, there are special laws allowing lenders to deal with you with regards to a HAMP modification or a HARP refinance. If your lender wont assist in a HARP refinance, it's because they choose not to do and not because they can't.

What makes you think you no longer legally own the property?  If you filed a Chapter 7 case and received a discharge and your case was closed without the Chapter 7 Trustee liquidating any assets (such as your property), then you still own it unless and until it is sold--either through a foreclosure or other sale. You are responsible for "keep up" fees as well as for any injuries which may occur on the property, so it's important to maintain your insurance until the property is actually sold. Your facts are unclear, but if what you're saying is that your mortgage lender has not yet foreclosed, then you should take steps to expedite that, such as offering to do a deed in lieu of foreclosure.
