A judgment was entered. It was then vacated on someone's motion (usually the defendant's). The case was then dismissed with prejudice, which means it cannot be refiled. If you did not file motions seeking such relief, it was most likely because the judgment was paid.

I have no idea, but the Judge is not a rubber stamp, and I am sure she had a reason. There may have been an error in the paperwork, that happens more than you would know sometimes as simple as he wants it done way x not way y. It could have been the 'paid extra' part the Judge may have had a problem with too.
