Look for the form at your local bankruptcy court website. I do not believe you will be able to find one that fits what you need. What you need is to amend the schedules to include the creditors that were by mistake not listed. Have you had your 341 meeting yet? If not, the trustee might be able to tell you what you need to do or point you in the right direction.

If you are unable to make your payments under a Chapter 13 repayment plan, the trustee may request the court to dismiss your case and your creditors will be allowed to continue collection efforts against you. It may be a better idea to convert your case to Chapter 7 before the case is dismissed.

The creditors will come after you if there are joint accounts for payment in full. If they are her separate debts you are okay. Each state has different exemption laws contact a lawyer where you are buying the house.
